I just watched it two days ago with my SO. I already knew about the twist, but she had never even heard of the movie (I asked her several times to make sure she really didn't know it). There were a couple of scenes where I kind of peered over to her and thought "Shit, she's gonna find out now", like when the kid gives his "I see dead people" line and the camera focuses on Malcolm very intensely. I read that the filmmakers thought about cutting this part because it might be too obvious and spoil the twist.
There were also a couple of other scenes where I wondered if I would have figured it out myself without any prior knowledge. Some key scenes were the basement door being closed off, the restaurant scene, the fact that he and his wife never talk or even look at each other. Generally I'm always thinking about Chekhov's gun when watching movies and I try to think of possible reasons why certain things are emphasized seemingly without any obvious relevance (like the locked basement door).
So my question is: did anyone here figure it out during their first watch? If so, I think it would also be interesting to know which year you first watched it. Maybe in 1999 it was much easier to pull such a thing off.
